Looking for RN’s with Home Health, Skilled Nursing and/or Case Management experience with an adult/elderly population. The position is full time with travel within the Northern California - mostly the Greater Sacramento Area. Flexible schedule.

Requirements

• Must possess a current, unencumbered, active license to practice as a Registered Nurse in the State of California.
• Reliable Transportation and valid driver’s license and insurance
• Must be knowledgeable of nursing and medical practices and procedures, as well as law, regulations, and guidelines that pertain to long-term and community based care.
• Must be able to work both independently and collaboratively with others in a team environment
• Able to function effectively in a fast-paced environment.
• Must demonstrate a high level of verbal, writing and listening skills.
• Computer literacy including use of email, Microsoft Word, Excel
• Demonstrated commitment to the provision of services for the under served and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically and psychologically impaired.
  
  

Essential Responsibilities

• Meet with each client individually and conduct an intake assessments and semiannual reassessments.
• Represents and/or advocates for and with clients in obtaining services.
• Collaborate with the client/patient and care team in the development of an individual service plan and support client/patient in the execution of the plan.
• Attend regular meetings with management and team.
